Thank you for your interest in our facility. We are very proud of the care we offer and strive to provide the highest quality of life for our residents.
To apply for placement in our home, contact us and we will send you an application form. Note that we are required to have a PRI/Screen (Patient Review Instrument) to review for admission. The PRI/Screen is a standardized New York State Department of Health (NYS DOH) form. These forms are generally completed by the hospital if you are awaiting placement while hospitalized, or by the County Public Health Nurse if you are residing at home or an adult home.
